If your student will be taking prescription medications at school, now is the time to think about getting the necessary paperwork filled out and signed by the ordering physician. We must have the completed paperwork in the health office before we can administer the medication to the child at school. Always remember, prescription medication must be brought to school in the original bottle from the pharmacy and needs to be counted and verified by the RN prior to the child receiving it at school. This is to include any emergency medications, such as Epi pens and inhalers. .
For over the counter medication, we also need the required paperwork in the health office prior to the child receiving the medication, although we do not need a physician signature, only the signature of the parent or legal guardian. The OTC medication must be brought in the original container, (not in an envelope or baggie), with an expiration date that will outlast the course of therapy. The RN will also verify the OTC meds prior to administration to your child.
If your High School student will be carrying their quick relief, emergency medication (epi pens or inhalers) with them throughout the day, there is paperwork needing to be obtained by the ordering physician that allows the student to carry this medication on them, after demonstrating that they understand and are able to administer the medication appropriately per the physician. Please also ask your physician to provide us with an Asthma Emergency Plan or an Anaphylaxis Emergency Plan.

These forms must be obtained new prior to each school year, so if your child had a plan before, we will need a new one for the 24-25 year.
