Dental Screenings with optional Fluoride Treatments will be offered by Salina Family Healthcare Dentistry at your child's school for all students of USD 327 on September 20th and 21st. If you DO NOT wish for your child to be screened, you must sign one side of the following consent form. If you wish for your child to be screened, you do not have to return the form UNLESS you want the additional no cost fluoride treatment done, then you must fill out, sign, and return the "consent for fluoride application" on the back side of the form.. These forms will also be sent home with your child in the next few days and must be returned to the office before September 1st.
